FORMS: Subscriber Form
Create forms to collect email subscribers and add them to your marketing audiences.
What is a Subscriber Form?
A Subscriber Type form lets you gather contact information and add people directly to your Email Marketing Audience—they don’t even need an existing account on Unificus. You can use this to build mailing lists for newsletters, announcements, event promotions, and more.
Important note: While both known and unknown contacts may complete a Subscriber Form, their information won’t create or update CRM profiles. The form’s sole function is to enroll them in your chosen Email Marketing Audience.
How do I create a Subscriber Form?
Start by selecting Forms from your navigation menu, then choose +Create Form.
You’ll have the option to select Subscriber Form from the list. You can also click the icon next to the +Create Form button to access the same options via dropdown.
Once selected, you’ll enter the form-building interface.
Give your form a name in the Title field. By default, “Untitled Form” appears here.
Add Headers and Footers
You can place custom HTML content above (Header) or below (Footer) your form.
Click the arrow next to either section to expand the HTML Content Block, where you can add text, images, or other media.
If you prefer to include CSS styling, click the CSS Class option.
Building Your Form Fields
Use the dropdown menu to select and add Form Fields. Since Subscriber Forms have no pre-configured fields, each one you add requires custom setup (similar to creating a new Custom Field).
You can also insert HTML Content Blocks through the Advanced Blocks dropdown for additional flexibility.
Click the expand icon next to any field, then select Edit Content to customize it.
A WYSIWYG editor opens for formatting. Switch to HTML mode by clicking the <> icon if needed.
You also have Embedded Options available. Toggle these to hide content in embedded contexts or display content only when Staff Submit.
Select Your Audience
Add to Audience(s): Choose which Email Marketing Audience receives these new subscribers from the dropdown.
Important: Only standalone Audiences are available here. Audiences synced to Circles cannot be selected.
You may also toggle ‘Set as default for this Audience’ to automatically associate this form with the Audience in your Marketing settings.

By default, submitted form data automatically enrolls the person in your selected Audience.
Enable Email Confirmation (Optional)
If you want subscribers to confirm their signup, turn on Enable email double opt-in.
A default confirmation message will appear in the editor. You’re welcome to customize the text and insert dynamic placeholders by clicking the {x} icon.

Important: The confirmation message must include the {{confirmationLink}} placeholder so subscribers can verify their email address.
Set Up Notifications
You can send email alerts whenever someone submits the form. These notifications can go to your staff, specific staff roles, or to the form submitter themselves.
