Understanding Forms in Unificus
Forms are a powerful tool for bringing new users into your CRM and gathering the information you need to operate your business. Each form type lets you design custom fields using a combination of standard fields, your own custom fields, and advanced blocks—such as HTML sections, multiple-choice options, and booking capabilities.
Which Form Type Fits Your Needs?
Unificus offers six distinct form options, each designed for specific use cases.
Kickoff Form
Use a Kickoff Form when you’re ready to launch a new relationship or sales cycle. These forms are designed to be placed on the public web through a direct link or embedded on your website, making them accessible to anyone.
When someone submits a Kickoff Form, you can configure automatic actions—such as assigning a role, selecting a coordinator, adding them to a Circle, applying generators, launching an onboarding workflow, or even requiring payment before portal access. Each Kickoff Form can be customized independently, letting you create multiple versions tailored to different scenarios.
Existing contacts can also use Kickoff Forms to refresh their details or initiate new workflows.

Checkout Form
Checkout Forms streamline the buying experience while automating your operations. Visitors can browse and select from items, add-ons, and packages you’ve configured, with the total price updating instantly as selections change. Built-in payment processing lets customers complete their purchase right away.
Checkout Forms work on your website, inside the portal, or via a direct link.

Update Form
An Update Form is intended strictly for contacts already in your CRM. Unlike Kickoff Forms, these require authentication—they’re never publicly accessible.
When an authenticated contact submits an Update Form, their CRM profile is refreshed with any new information. New data replaces what was previously saved. Because these forms require login credentials, you always know who’s submitting and can trust the data quality.
If you need a public-facing form, choose a Kickoff or General Form instead.
Support Form
Support Forms gather details needed to create support tickets. The support ticket system helps you manage customer questions and concerns efficiently, tracking them from submission through resolution. Your team handles tickets from the portal, while contacts engage through the portal, email, or email only—whatever you prefer.
While Support Forms live in your Support menu, they also have shareable links and embed options for your website, landing pages, or direct distribution.
When submitted, a copy goes to your Support Inbox and to the email address configured in your Support settings.

General Form
A General Form is a flexible, all-purpose form that doesn’t connect to specific contacts or companies. It works anywhere—inside or outside the platform, whether users are logged in or not.
General Forms collect information and send it to you or your team via email. Submissions are stored in the form’s records for your reference, but they won’t be linked to any contact or company in your CRM.

Subscriber Form
A Subscriber Form lets you build an email marketing audience without requiring people to be platform users. Use it to grow your newsletter list, share product updates, or send event invitations.

Switching Between Forms
While editing, switch to a different form by clicking the dropdown in the top right—it displays the name of your current form.

Viewing Form Submissions
Access submissions in two locations:
In a Contact’s Profile
Once you’re viewing a contact’s form submissions, use the arrows to filter by ID, date, name, or form type.
From the Forms Menu
Click the three-dot menu next to any form and select ‘Submissions’ to see all entries for that specific form.
