Appointments with Kickoff Forms

Enable contacts to book appointments through registration forms using Unificus Kickoff Forms.

Can my contacts book appointments through a form?

Absolutely! Using Kickoff Forms, you can create booking forms that allow your contacts to schedule appointments. This functionality works for both new contacts entering your system and existing users already in your database.

Contacts can set up an appointment as part of their initial signup, or established users can complete a form to reserve an appointment slot. They’ll select from available Appointment Generators (previously called Appointment Types) just as they would through standard appointment scheduling.

How do I build a Kickoff Form that includes appointment booking?

Start by going to the Forms menu and select +Create Form.

You can choose Kickoff Forms from the menu or use the dropdown icon next to +Create Form.

When you designate your form’s Role as either Prospect or Client, both new and existing contacts get the option to reserve an appointment during registration.

Pick Appointment Block from the Advanced Blocks menu.

The Block will appear in your list automatically. Use the dropdown to set the Time Zone for new contacts.

An Appointment Scheduling section will display in your form’s Assignments & Automations area.

Pick one or more Appointment Generators from the dropdown—they must already exist and have Active status. You can also decide whether booking an appointment is required to complete the form submission.

Drag and drop this field to position it anywhere in your form layout.

Important: If you deactivate or remove an Appointment Generator, it will automatically disappear from the form. Any updates you make to the Appointment Generator will instantly update the form as well.


Using a Choice Block? How do I set appointment options per choice?

With a Choice Block, your form’s registration details split across multiple choices. You’ll need to configure the Appointment Generator separately for each choice.

Expand the Choice Block by clicking its icon if needed.

Click the icon for each Choice Sub Block to open its settings.

Once you select Prospect or Client for the Role, the Appointment Scheduling section appears where you can pick an Appointment Generator.


What does the contact experience?

While completing the Kickoff Form, contacts see a button labeled Click to schedule an Appointment. Next to it appears a customizable message (editable through your Calendar Settings) that prompts the action.

Clicking the button opens a window where your contact selects their preferred Time Slot.

After picking a slot, the date and time display in place of the original message.

Once registered and logged into the portal, contacts view their appointment by going to Calendar > My Calendar.

They can click the appointment to review all the details.

The staff member assigned to that Appointment Generator also sees it on their calendar and can view complete details. They can also configure Reminder Notifications for themselves and the contact.

I added an Appointment Block but the booking option isn’t appearing!

For the Appointment Block to display on your form, two conditions must be met:

  1. The contact’s Role must be set to Prospect or Client
  2. An Appointment Generator must be selected in the Appointment Scheduling section

Without a Choice Block: Find the Role field in the Appointment Scheduling area within Forms Assignments & Automations at the bottom of the page.

With a Choice Block: This section won’t appear at the form level. Instead, configure each Choice Sub Block individually.

Click the icon beside each Choice Sub Block to access its settings.

You’ll find the Role field and Appointment Scheduling area inside the modal.
