FORMS: Form Field Types

Learn about the different field types available when building forms in Unificus.

What are Form Field Types?

Every field you add to a form has a specific type that controls both its appearance and how users can input data. The field type you choose determines the format users will see when they complete the form.

In Kickoff Forms and Update Forms, you can use Default or Custom Fields. These special fields are reusable across the entire Unificus platform, not just in forms.

Understanding Each Field Type

How your form fields display depends on which type you select when creating them. You can build both Custom Fields and Form Fields directly during form creation. You can also set up Custom Fields separately through Flyout Menu > Custom Fields.

Single-line Text Area: Best for brief answers—a single line for short responses, words, or numbers.

Multi-line Text Area: A larger box-style field for longer responses, paragraphs, or bulleted lists.

Email: Validates that responses follow standard email formatting.

Checkbox: A single checkbox for straightforward yes or no answers.

Date: Displays an interactive date picker so users can select their preferred date.

Dropdown: Opens a menu of predefined options. You can allow users to pick multiple selections if needed.

Multiple Choice: Shows a list of options, each with its own checkbox. Multi-selection can be enabled.

File Upload: Lets users attach files by dragging them into a designated area or clicking to browse their device. You can permit multiple file uploads.

URL: Validates that responses are properly formatted web addresses.

Number: Only accepts numeric input.

Name Prefix: A single-line text box where users can enter their preferred title or prefix.
